A program to prevent young people becoming homeless. Youth aged 12-18 experiencing difficulties at home or school are assisted and engaged in program specific projects targeting mental health issues and connectiveness to family and community. It is not a crisis intervention service or activities based service such as a youth centre. Programs are facilitated with existing and new clients and referrals are accepted from schools community members other agencies and self. Reconnect is a confidential free and voluntary service.
